feat: add operation registry foundation - #53

Summary

• add typed operation registry descriptors for the 16 analysis operations
• key MCP hints by OperationName while preserving existing tool-name lookup
• add registry unit coverage plus CH-P1-01 registry -> CLI JSON -> MCP JSON overview parity
• sync architecture docs, llms-full, and roadmap with foundation/remaining adapter work

Validation

• pnpm lint
• pnpm typecheck
• pnpm build
• pnpm test (30 files, 437 tests)
• pnpm verify:cli-real (56 pass, 0 fail)
• code-review pass: fixed one docs accuracy issue before PR

Release

• 2.5.0 canary train only; no stable release
